Fonus
One the natural incident of the human life is death because all of us will come to an end. Thus we provide a product that will deprecate the mourning of the family of the deceased through cash assistance and funeral service package. Fonus will ensure that they will provide quality funeral services and also cash assistance.

INDIVIDUAL PROGRAMS

  • At least 18 to 65 years old of age (Enrollment Entry Age)
  • Physically able to perform the usual or normal duties of his livelihood
  • Renewal up to age 66 years old us less 10% of the premium package. (Senior Citizen)
  • No medical examinations required
  • Plus additional cash Benefit
  • Yearly renewal (Fiscal Year). Maturity date depends on the date of enrollment
  • (Six) 6 months contest ability
  • Renewable every year

FAMILY PLAN

  • At least 18 to 65 years of Age (Enrollment Entry Age)
  • Physically able to perform the usual or normal duties of his livelihood
  • Renewal up to age 66 years old is less 10% of the premium package and will fall into Senior Citizen Program
  • No medical examination required
  • Plus additional cash benefit
  • Yearly renewal (Fiscal year).Maturity date depends on the date of enrollment
  • One year contest ability. (Failed to renew the annual premium shall be good as new and follow the procedures accordingly.) On the other hand, case to case basis will apply as the need arises, Fonus Cebu may extend help for those who are qualified after verification of documents submitted
  • Only mercury package is available for Family Program but the plan-holder can upgrade the casket by paying the difference of the casket cost

  • Legal Spouse (Husband or Wife)

  • Children
  1. At Least one(1) month old to age 21
  2. Maximum of 3 children
  3. Unmarried  and fully dependent to the principal
  4. Not a principal member
  • Children with Disability
  1. Physical and /or  mentally incapacitated
  2. Regardless of age
  3. Dependent upon the principal
  • Both Parents
  1. Not over 65 years old
  • Children
  1. At least one(1) month old to age 21(single)
  2. Dependent to the principal
  • Children with Disability
  1. Physical and/or mentally incapacitated
  2. Regardless of age
  3. Dependent upon the principal

  • Both parents
  1. Not over 65 years old
  • Children
  1. At least one(1) month old to age 21(single)
  2. Dependent to the principal
  • Children w/ disability
  1. Physical and/or mentally incapacitated
  2. Regardless of age
  3. Dependent upon the principal

SENIOR CITIZEN PROGRAM

  • Existing members who never failed to renew and reached the age of 66 years old will get a 10% discount on renewal of annual premium and no changes on the package.
  • New enrollee that already 66 years old or above will pay full premium package and can only avail the funeral service without cash assistance. Only mercury package is available for new enrollee (senior citizen) but the plan –holder can upgrade the casket by paying the difference of the casket cost.


Scope of Memorial Services:

  • Pick up of Cadaver
  • Preservation of Cadaver good for 7 days 
  • Set-up for viewing(Home/Funos Cebu Cebu*)
  • Preparation of Interment
  • Assures funeral cortege leaves on time as scheduled
  • Provides funeral coach to transport from viewing to interment site (25km free) and a fee of Ph.100 Per km will be charge in excess of 25km free.
  • 1 wreath flower arrangement stand.


Requirements of death claim:

  1. Claim Statement(Pro-format)
  2. Original or Certified true copy of death certificate
  3. Birth certificate or baptismal certificate or affidavit of two disinterested persons
  4. Birth certificate of beneficiary and two(2) valid ID’s
  5. Marriage Contract or baptismal certificate or affidavit of two disinterested persons 
  6. Police report in case of an accident
